Rooted in Guanling, reaching the world: The 10th World Congress on Integrative Medicine – Forum on Traditional Medicine and Cancer Rehabilitation (Co-Chair: Huang Yuhe) (Original speech)
Dear leaders, distinguished experts and colleagues, and friends from all sectors of society at home and abroad who care about and support the cause of integrative medicine and traditional Chinese medicine: Hello everyone! This golden autumn brings together the most accomplished minds and fosters the exchange of new knowledge. It is a great honor for me to attend the 10th World Congress on Integrative Medicine in my capacity as co-chair, joining medical professionals worldwide in this virtual gathering to celebrate this significant event, discuss the broader trends in the development of integrative medicine, and explore ways to safeguard human health. First, on behalf of myself, I would like to extend my warm congratulations on the successful convening of this congress, and I would like to express my sincere respect to the pioneers and colleagues who have long been deeply committed to the field of medicine and dedicated themselves to safeguarding public health.

I am Huang Yuhe. Born into a family with a long tradition of traditional Chinese medicine, my ancestors inherited the lineage of imperial physicians from the Southern Song Dynasty; this legacy has been passed down through the generations, with each successive generation devoted to the practice of medicine, spanning thirty generations and more than eight centuries. Over eight hundred years of continuous medical practice and thirty generations of compassionate inheritance, for me, this represents not only the continuation of our family lineage but also a profound medical mission and a lifelong entrustment of life that spans past and present; moreover, it forms the very foundation of my lifelong dedication to traditional Chinese medicine and my commitment to practicing the philosophy of integrative medicine.

This conference is centered on integration, driven by innovation, and guided by the original aspiration of benefiting the people; it serves as a high-level platform for the integration of traditional Chinese and Western medicine and the exchange of wisdom between the past and the present. Today, drawing on the experience passed down through generations and decades of clinical practice, I would like to share with my colleagues several reflections and insights centered on three core dimensions: preserving the fundamentals while treating diseases, establishing medical practice through therapeutic efficacy, and passing on the roots and lineage.
I. Treating patients while retaining them and placing people at the center of care: avoid surgical intervention whenever possible in oncology diagnosis and treatment
Throughout my many years of clinical practice, I have consistently adhered to one core principle: When treating tumors, I will avoid surgical intervention unless absolutely necessary. This is by no means merely a matter of subjective experience; rather, it reflects the prudent approach of a physician grounded in the holistic and balanced perspectives of Traditional Chinese Medicine, and more importantly, embodies the human-centered core philosophy of integrative medicine.
Modern medicine generally focuses on treating disease by targeting the physical lesion, whereas Traditional Chinese Medicine approaches disease from a holistic perspective, tracing its origins to identify the underlying cause. A tumor has never been merely a localized mass; rather, it is the consequence of an internal environmental imbalance that develops over time as a result of long-term imbalances in yin and yang, stagnation of qi and blood, dysfunction of the zang-fu organs, and deficiency of vital qi. The human body is an organic whole; behind any local lesion lies a systemic disruption of bodily functions and an imbalance in qi, blood, and body fluids.
The scalpel is precise and efficient; it can rapidly excise visible lesions and relieve acute crises, making its clinical value irreplaceable. However, we must maintain a clear awareness: While the lesion is removed, the root cause of the disease remains unaddressed; while a mass is excised, the diseased body's imbalance remains uncorrected. If treatment merely addresses the symptoms without treating the root cause—failing to regulate the visceral organs, replenish vital energy, and unblock the flow of Qi and blood, and failing to improve the internal environment in which tumors develop—then even after the lesion is removed, the risk of recurrence and metastasis persists. Furthermore, surgical trauma depletes the body's vital energy, leading to decreased immunity and physical debilitation, which conversely hinders subsequent recovery. The issue is that we must be vigilant against the gradual development of multiple lesions; every disease progresses from the exterior to the interior, from small to large, from mild to severe, and from benign to malignant. The Eight Factors that lead to tumor formation: deficiency, cold, dampness, stagnation, stasis, obstruction, tumor, and carbuncle (cancer). Cancer is formed by the Five Toxins; without the formation of these five toxins, tumors cannot develop. The Five Toxins are: cold toxin, heat toxin, phlegm toxin, Qi toxin, and blood toxin. If a virus invades and the body begins to decline, the situation becomes uncontrollable; therefore, extreme caution is required—and one must remember: do not use a knife! Biopsies and surgeries both involve intentional instrumental injury, which disrupts the circulatory system and destroys the body's balance! Once the micro-megaoles, micro-vascular channels, micro-neurons, and micro-vessels are severed, the circulation is broken; it can no longer be reconnected or restored. During this period, if combined with a full course of electrotherapy and chemotherapy, in which country, which hospital, and which doctor in the world can reconnect these four micro systems? The skin and flesh may regenerate, but the four micro systems can never be reconnected. When the four micro-systems are blocked and circulation is obstructed, the local incision where tumor tissue was removed releases exudate that accumulates as toxic fluid. We know that stagnant water smells; how much more so the fluid from a tumor surgery incision? In the future, this will lead to a continuous stream of sequelae, endless disasters, and immense suffering. This explains why the ancestors of Traditional Chinese Medicine opposed surgical treatment for tumors for thousands of years; moreover, there is no single, handed-down manual on tumor surgery in TCM. Are TCM practitioners simply that foolish? Tumor formation occurs when the six pathogenic factors of nature invade the human body, becoming the six pathogenic agents; these six pathogenic agents then transform into the five toxins (cold toxin, heat toxin, phlegm toxin, qi toxin, and blood toxin). Without the five toxins, tumors cannot form; without cold toxin, tumors are even less likely to develop. Frequently consuming cold-natured foods or cold-natured medicines in daily life—including acupuncture and herbal preparations—can readily lead to the accumulation of cold toxin. When cold causes coagulation and stasis, the condition becomes more obstructed the colder it gets; the greater the obstruction, the more toxic it becomes, and once toxins accumulate, disease manifests. Given sufficient time, a tumor will eventually develop.
In the treatment of diseases through Traditional Chinese Medicine, the core principle is that the treatment of a disease must address its root cause. When dealing with tumor-related diseases, we consistently adhere to a diagnostic and therapeutic approach that takes the whole body as the guiding principle and addresses both the symptoms and the root cause. Building on the core system of syndrome differentiation and treatment, and employing diverse methods such as the Nine Integrated Methods of Huang's TCM, we bidirectionally regulate the body's yin and yang, promote the free flow of qi and blood throughout the body, restore the functions of the zang-fu organs, and replenish the body's vital qi.

In clinical practice, traditional Chinese medicine is involved throughout the entire process of cancer prevention and treatment, resulting in a mature integrated diagnosis and treatment system. For patients with advanced-stage disease, the approach prioritizes strengthening the body's vital qi while supplementing it with the elimination of pathogenic factors, thereby enabling patients to live with the tumor, improve their quality of life, and extend their survival period. Consequently, the principle of avoiding surgery unless absolutely necessary does not mean rejecting Western medicine or denying the value of surgery; rather, it embodies reverence for life and prudent treatment. The core of integrative medicine lies in abandoning the mindset of relying on a single treatment modality, integrating the strengths of both Chinese and Western medicine, and giving priority to preserving the body's fundamental functions. By focusing on restoring the body's intrinsic balance and activating its self-healing capacity as the foundation, it aims to achieve the optimal therapeutic outcome of coexistence between the patient and the tumor, longevity with underlying disease, and dignified living. This represents the highest ideal of using comprehensive medicine to prevent disease before it arises and to prevent disease progression once it has developed.

II. Prioritize therapeutic efficacy and use practice as the benchmark; validate medical value through clinical outcomes
Medical value is measured through clinical outcomes; in medicine, there is no distinction between ancient and modern times—therapeutic efficacy is the sole definitive criterion. Whether it is the time-honored practices of traditional Chinese medicine or cutting-edge modern Western medical technologies, the ultimate goal of all medical theories, diagnostic and therapeutic approaches, and inherited techniques is to relieve disease and suffering, safeguard health, and benefit all living beings. Therapeutic efficacy is the foundation upon which medicine is built and the core benchmark for the development of integrative medicine.
Traditional Chinese medicine has endured the vicissitudes of thousands of years and withstood the test of the ages; it has continued to thrive and passed down its legacy from generation to generation. Its essence lies in its ability to withstand clinical practice, withstand the test of time, and withstand validation by patients. The classical prescriptions, principles of syndrome differentiation, and acupuncture techniques passed down through successive generations are not mere theoretical statements found in textbooks; they are the crystallization of wisdom refined through years of daily clinical practice by countless predecessors, serving as proven remedies that have healed countless patients.

In this new era, the spectrum of diseases is continuously evolving, complex and refractory conditions are becoming increasingly sophisticated, and public health demands are steadily rising; these factors place higher demands on the development of integrative medicine. To remain committed to the principle that therapeutic efficacy is paramount, we must uphold two fundamental missions: First, uphold fundamental principles and act with steadfast dedication, while deepening clinical practice. We must remain rooted in the front lines of clinical care, stay connected with patients, avoid empty theoretical discourse, and refine our clinical skills through the meticulous study of complex cases, the adaptive application of traditional methods, and the optimization of treatment protocols. This ensures that the wisdom of Traditional Chinese Medicine (TCM) aligns with the needs of modern disease diagnosis and treatment, and that time-honored prescriptions serve the health needs of the contemporary public. Second, embrace openness and inclusivity, and keep pace with the times. The essence of integrative medicine lies in its capacity for synthesis and the leveraging of complementary strengths. We will not remain stagnant or rigidly adherent to outdated conventions; instead, we will proactively integrate with modern medical diagnostic technologies and evidence-based medicine frameworks. We will elucidate the mechanisms of TCM using modern scientific language and validate its efficacy through objective clinical data. By transforming our experience in individualized syndrome differentiation into replicable, verifiable, and scalable medical achievements, we can transform the perceived benefits of TCM from subjective patient experience into scientifically proven, industry-recognized core capabilities. This will allow traditional medicine to truly integrate into the modern medical system and gain global recognition. Only by prioritizing efficacy as our guiding principle and grounding our practice in real-world application can the integration of Chinese and Western medicine be truly realized, the wisdom of the past and present truly converge, and the field of medicine genuinely benefit the people and achieve sustainable, long-term progress.

III. Tracing our origins and preserving our roots, passing the torch forward, and revitalizing the millennia-old tradition of medicine for the modern era
Traditional Chinese Medicine (TCM) is a treasure of China's outstanding traditional culture and a precious legacy left to future generations by physicians throughout the ages; it possesses history, roots, soul, and warmth. The principle of promoting historical continuity stems from the fact that TCM has a long-standing cultural lineage and a comprehensive system. From the *Huangdi Neijing*, which laid the theoretical foundation of TCM, and the *Shennong Bencao Jing*, which established the pharmacological system, to the *Shanghan Zabing Lun*, which established the criteria for clinical syndrome differentiation, the millennia-old classics have preserved not only diagnostic and therapeutic techniques but also a philosophy of life centered on the unity of humanity and nature, the balance of Yin and Yang, and a people-oriented approach. This constitutes the cultural foundation for those who practice TCM and serves as the ideological bedrock for the integration and convergence of integrative medicine, as well as for harmony and therapeutic practice. The notion that promoting historical continuity and ensuring rooted transmission means that TCM culture has never been a rigid set of theories locked away in dusty ancient texts; rather, it is a medical system characterized by living transmission and dynamic development. Its roots lie in the passing of knowledge through oral instruction from master to disciple, in the clinical intuition of syndrome differentiation and treatment, and in the diagnostic wisdom that embodies the unity of heaven, earth, and humanity. Without transmission, TCM loses its soul; without roots, medical innovation becomes like water without a source or a tree without roots. In the new era, the transmission and development of TCM and the advancement of integrative medicine require us to both preserve the classics in their original form —deeply studying the cultural lineage of ancient texts, adhering to core clinical principles, and passing on distinctive skills to safeguard the essence of the millennia-old medical tradition—and to keep pace with the times to cultivate new generations —training a cohort of physicians in the new era who understand the classics, are proficient in clinical practice, excel in integration, and possess the capacity for innovation, thereby ensuring that the orthodox medical lineage is passed down from generation to generation and continues to thrive.

Colleagues and friends! Integrative medicine represents an inevitable trend in the development of medicine; upholding fundamental principles while pursuing innovation is the only viable path toward the revitalization of traditional Chinese medicine. Standing at the new starting point of the 10th World Congress on Integrative Medicine, I would like to join all colleagues in remaining true to our original vocation as medical practitioners: to take the whole person as the foundation, exercise prudent clinical judgment, honor life, and overcome complex and refractory diseases; to make therapeutic efficacy the core, deepen clinical practice, pursue truth and pragmatism, and demonstrate the value of medicine; and to take heritage as the root, uphold fundamental principles while exploring new approaches, integrate Chinese and Western medicine, and carry forward the millennia-old cultural legacy.
Let us stand side by side, pool our efforts, delve deeply into the field of integrative medicine, synthesize the wisdom of ancient and modern times, and embrace the strengths of both Eastern and Western medicine. We will promote the inheritance and innovation of traditional Chinese medicine and the high-quality development of integrative medicine, enabling the treasures of traditional Chinese medicine to take root in China and reach the world, thereby contributing the wisdom and strength of China to safeguarding the health of all humanity and building a community of health for all.
